From Dennis Reardon, Assistant Principal and Director of Athletics at Marple Newtown High School:

Bryn Mawr Family Practice will be at Marple Newtown High School on Friday, June 12th, from 11:00 AM until 1:00 PM to administer sports’ physicals. These exams will cover student-athletes for the entire 2015-2016 school year, unless an injury would occur at some point after the exam. Students should enter through the gym entrance on the side of the school. Exams will be administered in the gym area. Cost of the physicals will be $10 per student and cash is recommended.

Pages 1-5 are to be filled out by a parent prior to the exam being administered. Section 5, Health History, must be filled out by a parent or guardian and returned to the high school no later than 5/29/15. This will be forwarded to Bryn Mawr Family Practice for perusal in the event of any preexisting conditions.

Page 6 will be filled out by the physician at the time of the exam. Pages 7-8 are not necessary at this time. No student will be administered the exam without pages 1-5 being filled in by a parent prior seeing the doctor. A copy of your exam will be kept on file at the high school through the next year. In the event you would need a copy of the exam, contact the athletic office.
